The Uniform Dental Plan (UDP) offers you easy access to care from the provider of your choice with sensible coverage for the services you need.

You’re in charge—it’s that simple.

Your Uniform Dental Plan Highlights

Your Uniform Dental Plan (UDP) is a preferred provider organization (PPO) plan that gives you access to care through the Delta Dental PPO network. It’s the state’s largest network, with more than 3,400 dentists.

UDP gives you the freedom to see any licensed dentist. However, you’ll usually receive a higher level of coverage, lower out-of-pocket cost and no claims paperwork when you see a Delta Dental PPO network dentist.

Your UDP plan also:

Covers 100% of preventive services
Allows you to see a specialist without referral
Gives you coverage when you travel outside Washington State

	Uniform Dental Plan Preferred Provider Plan
Annual Deductible	$50 per eligible person each benefit period; $150 family deductible maximum
Maximum Annual Plan Payment	$1,750 per eligible person, each benefit period
Procedure	PPO Dentists in Washington State	Out-of-State Dentists	Non-PPO Dentists in Washington State
	After deductible, you pay:
Cleanings and exams	0%	10%	20%
Fillings (Restorative)	20%		30%
Crowns (Restorative)	50%		60%
Orthodontia	50%
Tooth extractions	20%		30%
Root canals (Endodontics)	20%		30%
Treatment of gum disease (Periodontic services)	20%		30%
Dentures	50%		60%
Implants	50%		60%
Nonsurgical TMJ	30% of costs under $500 for PPO, out of state or non-PPO; then any amount over $500 in member's lifetime
Orthognathic surgery	30% of costs up to $5,000 for PPO, out of state, or non-PPO; then any amount over $5,000 in member's lifetime
