Our priority is to keep our employees healthy, while also supporting the needs of our members, dentists and communities. As a result of this situation and most recently the governor’s shelter in place order, you may experience extended hold times when calling customer service, or longer turn-around times on other correspondence or claims. To start the process of becoming a new member dentist with Delta Dental of Washington, please complete the forms located here. These forms provide us with the information we need to start the credentialing process and have your information updated in our system.

We'll let you know when your request has been processed and follow-up with you on the next steps. If you have more than one office location, please complete a separate Additional Treatment Office Information form for each location, this form can be found in the membership application packet above.
